Hi, Class of 2021, congratulations on making it to the end of the Winter Term! It will soon be time for Commencement, and we need you to verify the names on your diplomas tonight! (The deadline was two days ago)

Please us this Google Form–it displays the name that will currently be used on your diploma. If you would like to keep the name the same, simply check the box that indicates so. If you like to change* the name (such as adding or removing a middle name), please check the box that indicates the change and add your full name in the following section.

*Please note that any name-change provided should be appropriate for a formal document.
