SLO - Advisor Request for 2024 - 2025

The Student Life and Academic Affairs Office encourage you to select for an advisor, an adult on campus with whom you have already established a meaningful relationship. Existing dorm and advisee commitments may limit the availability of some faculty members, so please be sure to consider and select your top FOUR choices below and make certain to follow up with that adult in person. You must fill out this form, EVEN IF YOU WISH TO REMAIN with your current advisor.
