Ac Affairs - Course Change Request Form - Consulted Course Change (Preference or Error Change) 25-26

Form must be filled out by a Deerfield adult- teacher, advisor, college advisor.
Eligible courses: All Spring Term courses, within policy guidelines.
Requirements: If the course has already begun, students must have a face-to-face conversation with their current teacher(s) and then work with an advisor or teacher to submit the course change request.
