Transcript Request Form

Please complete the form below to request a copy of your Deerfield transcript. If you have attended another high school, you must request an official transcript directly from that school.Please note in submitting this form, you are authorizing Deerfield Academy to provide your transcript to the persons/agencies you list.

You may also use this form to request a copy of your NCAA transcript. (Please note that you MUST REGISTER with the NCAA PRIOR to COMPLETING THIS FORM. Also your test scores cannot be sent to the NCAA from this office. Deerfield will no longer be able to submit previous high school transcripts to the NCAA on your behalf.)
